  • If a failure analysis is needed, you must also complete that part of the form.

  • The completed form must be sent to Michelle Vega at TDK-Lambda

    (Michelle.Vega@us.tdk-lambda.com).

  • Once completed and returned to TDK-Lambda, an RMA number will be assigned and return instructions will be provided.
